…I really hate it when they use a film's main theme in the film itself as source music in bars, at parties or on the radio.
This definitely would detract from the realism of a scene — if it is realism that's being strived for. This sort of practice, which was much more common in the past than it is today, put the movie in its own little self-contained universe, which in some cases was the intended effect.

Don't forget the Ghostbusters dancing to their own theme at a kid's birthday party in GHOSTBUSTERS 2. It really bothered me then, and now.
That was the first example that came to mind when I saw this thread. Ray Parker, Jr. deliberately wrote the theme to resemble a television jingle more than an actual song, though, so it probably makes sense that they would have a jingle that runs with in-universe advertisements.
Then ... John Williams, which cuts to Williams' face which is wide-eyed, and stern, conducting. The "camera" pulls away and he's got a large orchestra, performing very, very, very serious porn music.
IIRC, it's actually Desert Chase from Raiders.
It wasn't the exact track, but the music heard is clearly an homage to "Desert Chase."
On a related Williams note (and I realize this isn't a film example, but thought I'd throw it in anyway), the "Imperial March" has actually been referred to within Star Wars Expanded Universe material as the "Martial Theme of the Empire," often played at Imperial Navy graduation ceremonies.

I'm pretty sure in an episode of "Batman: The Animated Seires", Harley breifly hummed her character's theme.
There was an episode of the short-lived TV seires "Vengeance Unlimited" ("Clique") where the star of the show, Michael Madsen, hums the main theme from the show.

One of my earlierst film score memories... I could whistle that tune afterwards, too, and I knew the name "Dimitri Tiomkin", the first film composer I knew by name.

The janitor scene brought a tear to my eye. Seriously.
In Tron Legacy Flynn sings the Wendy Carlos Tron theme (which is exactly one time more than Daft Punk used it in the score).
Did anyone mention the Star Trek theme being played as source music in Conscience of the King?
