I don't know about the rest of Europe, but there are tariff limits for items coming in from the U.S. I believe it's something like £18 (about $30 at current ForEx rates) before there is a duty payment to be made, so most lower value items won't attract interest anyway. This page explains it better:
Same in Norway. If an item (books excepted) costs over 200 NOK before shipping, we have to pay extra taxes. It's silly, because it's an archaic limit that was set 30 years ago, long before the advent of the internet. However, it's supposedly meant to protect Norwegian vendors.

Definitely. Last month I had to pay 50 € (about 75 dollars) for a dvd boxset I bought in the States because it got held up by customs. A case of bad luck, not everything gets held back.
How much? £8 (about $15) post office handling PLUS (from memory) 6.9% of the value of the parcel. That's assuming it gets stopped.
It's £8 handling + VAT on the entire value including the cost of postage where the item value is >£18 or >£36 in the case of a "gift" (but a gift must be sent from an individual, not a company).
