gotta go with old faithful- Dies Irae as presented in The Shining. Carlos' rendition with Rachel Elkind's eerie vocals is creepy as all get out.

Runners Up
Amityville Horror- Schifrin
The Omen- Goldsmith

Honourable Mentions:
The Conjuring- Bishara
Evil Dead- Banos

Totally agree with Carlos' The Shining main title cue. Bone chilling and disturbing. When the vocals kick in..shivers. Creepiest ever in my book, no matter how many times I hear it.

Jerry Goldsmith's main theme from "Seconds" comes to mind.

The chief seems a bit out of his mind. A dead woman lies on his desk. He claimed he had no choice because she was turning into a zombie. Claire leaves his office knowing that there's nothing more to be gained from him. Once she come across a means to proceed through the Raccoon Police Station, she returns to his office.

The chief and the body are gone...

Out of left field but I wish that a label would release the music for Midsomer Murders (the British murder mystery drama). One episode in particular ("Left for Dead") has some very sinister and frightening music which is also creeeeeeeeeepy.

I will add that the use of Bartok's Third Movement from Music for Strings, Perc and Celesta is still one of the creepiest pieces of music I've heard no matter how many times I've listened to it. That and Ligeti's Requiem.

With the Halloween season coming right up I thought I'd start this thread. For me it's the DARK SHADOWS theme, it has always creeped me out. What about you guys?

Can't agree more - scared the bejeezus out of me when I was a kid!

There's some music in The Assassination of Trotsky that's quite creepy. Whispering choir.

I love the slow, building menace of Carpenter’s Halloween

Without a doubt, THE HAUNTING (1963) composed by Humphrey Searle. Followed by Georges Auric for DEAD OF NIGHT (1945) and THE INNOCENTS (1961). I also agree with another poster about the TV show ONE STEP BEYOND, the theme music of which is available on CD, as is THE HAUNTING.

‘Don’t Be Afraid of the Dark’ by Billy Goldenberg. The melody and chord changes are really quite scary.

I like John Barry's 1966 re-recording of his Main Title (for CBS/Columbia) from Séance on a Wet Afternoon ... it is very eerie with very deep bass.

Amazed that Goldsmith's Magic hasn't yet been mentioned.

Likewise, Frontiere's Don't Go to Sleep.
