Probably said before but "The elephant in the room." It's so tired that it probably perpetuates rather than calls attention to some ignored major issue. It probably engendered the joke about finding elephant footprints in your refrigerator.
I nominate this replacement, "The shark in the pool." It adds urgency to the metaphoric problem, and anyone dragging their feet can be chastised with "I think that I am familiar with the fact that you are going to ignore this particular problem until it swims up and BITES YOU ON THE ASS!"
