I think there might be some wiggle room for what I'm about to say.
It's possible that Starfleet would not have sent a ship to check on Khan if they were under the impression that Ceti Alpha 5 was destroyed--not knowing that it was, in fact, Ceti Alpha 6 that "exploded".
This would seem to bear out when we see that Chekov and Terrell thought they were on Ceti Alpha 6 when they beamed down.

Plausible?

I'll buy that!

Kirk gets news of Ceti Alpha V blowing up in between The Changeling and Mirror, Mirror.

Kirk: "oh crap, Khan was on that planet. Damn it..."

14 and a half years later, Khan shows up on his view screen:

Kirk: (shocked that he's alive) "Khan!" (thinks) "wow, if I knew he was alive I would have bothered to check on his progress."

To be fair to Kirk and crew, everyone on the ship seemed to forget just about anything that happened in prior episodes, so I think Kirk can be forgiven for forgetting to inform Star Trek about leaving Khan and crew at Ceti Alpha V.

In all seriousness, if Kirk didn't inform Star Fleet of what happened, how did he explain what happened to Marla McGivers?

With a name like McGivers, could she have helped Khan escape Ceti Alpha V using duck tape and a paper clip?

Kirk, the man with the eye for the ladies, didn't find her memorable enough to remember her name, so Starfleet probably never noticed.
